One possible cause is Morton's neuroma, a condition involving irritation and thickening of tissue surrounding a nerve in the forefoot. Although it is not actually a tumor, Morton's neuroma can become increasingly painful if left untreated. What Is Morton's Neuroma?

Foot structure can also play a role. Bunions, hammertoes, flat feet, and high arches may alter weight distribution across the forefoot, increasing irritation around the nerve.

In cases where conservative care does not provide adequate relief, surgery may be considered to remove the affected nerve or relieve pressure on it.
